They are playing 'doctor'. Obviously, Nathan is sick, and Mia is the doctor.
These are some pictures taken of Mia and Nathan at Kayla's wedding. Mia is a cousin on the other side of the family, but her very talented Mother was there taking pictures for the wedding.
